The Efficacy and Safety of 5-day Cefaclor Therapy in the Acute Otitis Media




Abstract
To evaluate the efficacy and safety of 5-days of Cafaclor therapy in the treatment of acute otitis media(AOM), authors selected 90 patients(109 ears) with AOM using the following criteria ; 1) adults and children of either sex, 2) patients receiving no antibiotic therapy within the previous week, 3) patients without any systemic or significant underlying diseases, 4) patients having no allergy to cephalosporins, 5) patients having no -craniofacial anomalies, and 6) patients showing intact tympanic membranes.
The tympanocentesis and bacterial culture were performed after antisepsis of the ear canal by the instillation of 70% alcohol for one minute and flushing with an acetic acid-alcohol mixture(50 : 50). Cefaclor was given orally to the patients(adult : 1500mg/day in three divided doses, children : 40mg/kg/day in three divided doses) for 5 days and its efficacy was evaluated at 5 and 10 days after start of therapy using the following criteria ;
1) Complete resolution : clinically well and normal ear findings,
2) Clinical resolution : clinically well but persistent middle ear fluid, and
3) Persistent infection : persistent symptoms and physical findings of AOM
The compliance was classified as follows ; 1) good : correct amount of antibiotic taken, 2) poor : inadequate amount of antibiotic consumed, and 3) questionable.: medicine bottle was not returned. Bacterial culture revealed that the causative bacteria in descending order of frequency were Streptococcus pneumoniae(29.1%), Haemophilus influenzae(21.8%), Streptococcus pyogenes(9.1%), and Branhamella catarrhalis(3.6%). We were not able to culture any bacteria in 15 ears(27.3%)
Five-day Cefaclor therapy was effective in treating AOM in 88 cases(97.8%) and in addition,
complete resolution was observed in 72.3% at 10 days after start of therapy. Minor sidereactions of Cefaclor, such. as nausea and¢¥ skin eruption, were noticed in 3 cases.
Present results suggest that five-day Cefaclor therapy is effective and safe in the treatment of AOM.
